Please note that the following items must be submitted before student may be enrolled.

This process may be accelerated by bringing all information needed with you.

□      Proof of Residence (telephone or utility bill etc.)

□      Complete record of Immunizations (Washington State requirement)

□      Copy of transcript and if transferring mid-year a withdrawal sheet showing classes/grades of last          school attended.

□      HSPE scores or state exam scores

□      Attendance Report

□      Discipline Report

□      Copy of IEP:                Does student have IEP:             yes____     no_____

□      Copy of 504 Plan:  Does student have 504 Plan:      yes_____   no_____

Upon submitting the criteria above, you will be contacted to schedule an appointment with the registrar and counselor to complete the registration process.
